In short
Send the referral and we take it from there. Patients are usually scheduled within the week, EMG studies are read the same day they are performed by Dr. Sheikh, and the consultation note goes back to your office after the visit.
What to send
What makes a referral usable on the first pass.
- Patient name, date of birth and a phone number that reaches them
- Working diagnosis and the symptoms you want evaluated
- Imaging reports already done, and the films if you have them
- Insurance information, or the claim number and adjuster for an injury case
- Whether you are asking for treatment, for an EMG study, or for both
If a piece is missing, send it anyway. The front desk will call your office for the rest rather than hold the referral.
